Catriona Cameron has been sent to live in Edinburgh to learn how to be an earl’s sister-in-law. Her lover, the man she rebuffed, is incensed at her popularity in London, and attempts to kill her. Dina MacTavish, having been responsible for Catriona, is now concerned that she’s willing herself to die. She calls in Dr. Mark Thorburn, who agrees to treat Catriona. Since she has such an aversion to doctors (due to her treatment in London), he pretends to be a footman.

They have a love affair, interrupted by a fire that Mark thinks isn’t an accident. He begins to investigate, certain that Catriona knows something she’s not telling. She believes that Andrew is still after her. After a third attempt on her life, she discovers that Mark isn’t just a penurious doctor, but the heir to an earldom. She realizes that the Catriona she is now could never be his wife, and retreats once more to her rooms. Along the way to that realization, she changes in character, becoming kinder and more compassionate. One of those acts of kindness makes Mark know for certain that she’s not the girl he’d once known.

Happy ending, of course, when Mark once again becomes a footman, refusing to leave until she agrees to marry him.
